Thank you for your gift to Moorelands Kids!

By providing details of your gift using this form you help us to help kids more, and help us to process your request for a tax receipt quicker as well!

Please note the following:

1. Be sure to have all your receipts and donor information handy before you begin. Contact info and mailing address is required for each donor receiving a tax receipt.
2. You can specify any number of donors and provide details for any number of Holiday Sharing "Adopt a Family" families in one form submission.
3. You can now upload your purchase receipts - PDF files, scans or photos are supported. Prepare these in advance and make sure all pertinent information is in view and in focus.
4. The information you provide will be emailed to you after you complete this form.
5. You must print the email and include the printout in the same envelope as the physical purchase receipts you provide to Moorelands Kids.
