Thank you for your gift to Moorelands Kids!

By providing details of your gift using this form you help us to help kids more, and help us to process your request for a tax receipt quicker as well!

Please note the following:

1. Be sure to have all your receipts and donor information handy before you begin. Contact info and mailing address are required for each donor receiving a tax receipt.
2. You can specify any number of donors and provide details for any number of Holiday Sharing "Adopt a Family" families in one form submission.
3. Upload your purchase receipts - PDF files, scans or photos are supported. Prepare these in advance and make sure all pertinent information is in view and in focus.
4. The information you provide will be emailed to you after you complete this form.
5. If you are unable to upload digital receipts, then print the confirmation email and place in one envelope with ALL purchase receipts.
6. Write your name, family number(s) and confirmation code on the envelope and hand it to Moorelands Kids staff when you drop off your gifts.
